United Concordia Coverage Availability & Fees

United Concordia offers a number of different group dental insurance plans. A DHMO policy only provides coverage at specific care providers. If you have a PPO plan, your treatment may be covered at approximately the following rates:

  • Preventive care (such as regular exams and cleanings): 100% covered
  • Minor restorative care (such as fillings): 70% – 80% covered
  • Major restorative care (possibly including root canal therapy and crowns): 40% – 50% covered

Also, keep in mind that PPO plans enforce an annual maximum, which is the highest dollar amount that United Concordia will pay out for approved services within a calendar year. Visiting an in-network dentist, such as our practice, will help you to get the most out of your annual maximum. That is because we have pre-negotiated the fees for certain services with United Concordia. Out of network dentists do not have such a contract, so patients often end up paying significantly more out of pocket.
